Purchase Loyalty correction

Does anybody know where to find the form we have to submit for purchase loyalty corrections? I lost all files on my computer and that was one of them.

If you have MS Excel, go to Dealer Daily Report, download the Purchase Loyalty Report, and at the top right you can open the adjustment form, fill it out and save it to your desktop. Then email it to samedayadjuster @ gm dot com.

If you select the arrow in the column above NON-RIM Dollar Value and uncheck zero value items, all you'll have left are the penalized items.
