Certification Label

Has anyone had any luck ordering a replacement certification label? I have a customer that the label is missing, and DMV red flagged the truck. I found an order form, but they require a picture of the label. The only contact information is an email address, I sent an email 7/27/22 and no response.
If anyone has a phone# to call, or any other information to get a label Please Help.

NOTE NOTE NOTE! - You no longer need to provide pictures in most cases. You may order using part number PN#19355584.

For this part number, you just need to provide a VIN via the ordering the system.

If you have been directed to this form by Duramark, please follow the rest of the instruction below.

This form is step 2 in the process. The first step is to place an order for part #10284728 in the Genuine GM parts system.

In order to fulfill the order, a readable picture of the existing cert decal needs to be supplied.

It needs to be readable and show complete Cert decal.

This form must be filled out and picture attached before the order can be fulfilled.

I have had a label problem solved with the help of this gentleman at Duramark.

Maybe he can help you or direct to the correct party ??

we have used ecsvin(dot)com 1-855-532-7846
Pricey but good quality. We had to get one or else a vehicle would not pass state inspection.

Thank You, I finally heard back from Duramark, and sent a copy of VIS and RPO's He thinks that He has enough info to print a new label.
