GM hazardous material return

Question for the GM parts managers. I am doing a return on a pre tensioner and on the pc0302 under the hazardous declaration there is a column for D.O.T.Ex &SP info. What goes in that column? this is my first hazardous material return.

if it does not say on the haz mat site to put anything, i just leave it blank.

All I did was use the class 9 label with description and filled out the Bill of Lading for Central Transport, marked it hazardous with the proper description. And FYI, Central won't take it unless it's on a pllet, so yes you have to put a 1 lb box on a 65 lb pallet. The driver just laughed. But I talked to Central and that's what they told me. Only thing I forgot to do was write the emergency phone number on the bill of lading. It's not that hard really other than batteries I'm sure are more intensive.
