I have had the issue when my counter guys are in a hurry they forget to put the quantity in and shows on the ro without a quantity like a lost sale. They always say they put the quantity in, but I believe they just get in too much of a hurry. If this is not the case you might contact customer service when you have an active example for them to look at.
